How do I make a request?
Extension Request
If you need an extension on the date to appear on your ticket (provided on your courtesy notice), contact the clerk’s office by email, by phone, or in person.
Modification Request
If you need to modify an existing order, fill out the Modification Request Form SUPTMV-056 and submit the completed form by email, by mail, or you can drop off in person at the Clerk’s office.
