Requesting an Extension or Modification

To request an extension of time to appear or a modification of an existing court order, fill out the Modification Request Form SUPTMV-056 and include your name, address, and case number. You can submit the competed form by sending an email, by mail, or you can drop it off in person at the Clerk's Office.
